{"id":195,"date":"2019-04-12T11:51:19","date_gmt":"2019-04-12T15:51:19","guid":{"rendered":"http:\/\/stmaryrockledge.diocesanweb.org\/school\/?page_id=195"},"modified":"2020-08-21T13:18:19","modified_gmt":"2020-08-21T17:18:19","slug":"admission-requirements","status":"publish","type":"page","link":"https:\/\/stmaryrockledge.org\/school\/admission-requirements\/","title":{"rendered":"Admission Requirements"},"content":{"rendered":"\n<div class=\"wp-block-columns alignwide has-2-columns is-layout-flex wp-container-core-columns-is-layout-9d6595d7 wp-block-columns-is-layout-flex\">\n<div class=\"wp-block-column is-layout-flow wp-block-column-is-layout-flow\">\n<p class=\"has-medium-font-size\"><strong>Age Requirements<\/strong><\/p>\n\n\n\n<p>All children entering St. Mary Catholic School are subject to the policies of the Diocese of Orlando, Brevard County, and the State of Florida.&nbsp; Children entering the early childhood programs must be potty trained and meet the following age requirements:<\/p>\n\n\n\n<ul class=\"wp-block-list\"><li>Preschool 3-year-old must be 3 years old on or before September 1st of the academic year.<\/li><li>VPK must&nbsp;be&nbsp;4 years of age on or before September 1st of the academic year.<\/li><li>Kindergarten must be 5 years old on or before September 1st of the academic year.<\/li><li>Grade 1 must be 6 years of age, or have successfully completed Kindergarten at an \u201caccredited\u201d public or private school.<\/li><\/ul>\n<\/div>\n\n\n\n<div class=\"wp-block-column is-layout-flow wp-block-column-is-layout-flow\">\n<p class=\"has-medium-font-size\"><strong>Student Records<\/strong><\/p>\n\n\n\n<p>St. Mary School requires copies of the following documents<\/p>\n\n\n\n<ul class=\"wp-block-list\"><li>Current report cards \u2013 2 previous years (if applicable)<\/li><li>Standardized test scores<\/li><li>Medical or special needs records<\/li><li>Cumulative Record Request<ul><li>Our office will request all of your child\u2019s records to be sent from your former school to show that your child has been attending school regularly and is in good academic and disciplinary standing.<\/li><\/ul><\/li><li>Student Recommendation Forms \u2013 2&nbsp;forms<ul><li>To be completed by your child\u2019s last teacher, administrator, or guidance counselor. Full disclosure of a student\u2019s academic, behavioral, legal, and medical conditions or learning disabilities is a requirement for admission and continuation at St. Mary School.<\/li><\/ul><\/li><\/ul>\n<\/div>\n<\/div>\n\n\n\n<div style=\"height:41px\" aria-hidden=\"true\" class=\"wp-block-spacer\"><\/div>\n\n\n\n<p class=\"has-medium-font-size\"><strong>HEALTH AND IMMUNIZATIONS<\/strong><\/p>\n\n\n\n<p>St. Mary School complies with all health and immunization requirements of the State of Florida and the Diocese of Orlando. St. Mary School does not accept religious exemptions from families who choose not to have their children immunized. We can only accept Medical Exemptions that can be obtained from your family medical doctor.<\/p>\n\n\n\n<p>All new families will be required to provide the following at the time of registration:<\/p>\n\n\n\n<div style=\"height:20px\" aria-hidden=\"true\" class=\"wp-block-spacer\"><\/div>\n\n\n\n<p style=\"font-size:24px\"><strong>1. The official blue shot record showing proof of the required immunizations:<\/strong><\/p>\n\n\n\n<div style=\"height:20px\" aria-hidden=\"true\" class=\"wp-block-spacer\"><\/div>\n\n\n\n<div class=\"wp-block-columns alignwide has-2-columns is-layout-flex wp-container-core-columns-is-layout-9d6595d7 wp-block-columns-is-layout-flex\">\n<div class=\"wp-block-column is-layout-flow wp-block-column-is-layout-flow\">\n<p class=\"has-medium-font-size\"><strong>Entering Pre-K through 6th Grade<\/strong><\/p>\n\n\n\n<ul class=\"wp-block-list\"><li>Diphtheria, Pertussis, Tetanus (DPT) 5 dose<\/li><li>Polio Vaccine 4 doses<\/li><li>Measles, Mumps, Rubella (MMR) 2 doses<\/li><li>Hepatitis B vaccine series 3 doses<\/li><li>Chicken Pox 1 dose<\/li><\/ul>\n<\/div>\n\n\n\n<div class=\"wp-block-column is-layout-flow wp-block-column-is-layout-flow\">\n<p class=\"has-medium-font-size\"><strong>7th Grade or Above<\/strong><\/p>\n\n\n\n<ul class=\"wp-block-list\"><li>Hepatitis&nbsp;B series 3 doses<\/li><li>Measles, Mumps&nbsp;Rubella (MMR) 2nd dose<\/li><li>Tetanus-Diptheria&nbsp;Booster (TD) 1 dose<\/li><\/ul>\n<\/div>\n<\/div>\n\n\n\n<p style=\"font-size:24px\"><strong>2. Health examination form (Kindergarten and up) on file. Children will not be admitted into school without these required forms on file.<\/strong> <\/p>\n\n\n\n<div style=\"height:40px\" aria-hidden=\"true\" class=\"wp-block-spacer\"><\/div>\n\n\n\n<p style=\"font-size:23px\"><strong>3. Child\u2019s birth certificate<\/strong> <\/p>\n\n\n\n<div style=\"height:50px\" aria-hidden=\"true\" class=\"wp-block-spacer\"><\/div>\n\n\n\n<p style=\"font-size:25px\"><strong>SACRAMENTAL RECORDS \u2013 For New Catholic students<\/strong><\/p>\n\n\n\n<p>For new Catholic students, a copy of the baptismal certificate is required at registration. If you are applying for the Category 1 rate, then your child must be baptized Catholic or is in the process of being baptized.
